New Patient Registration
Please call the Rheumatic Disease Center at (414)-351-4009 ext. 21 to make a new patient appointment.
Please have the following information available when you call:
Legal, Full Name
Current Address and Telephone Number
Current Insurance Information
Emergency Contact Information
RDC will mail you the appropriate paperwork which we must receive completely filled out before your new patient appointment. This will give our staff the opportunity to thoroughly review and accurately serve your needs before your appointment.
Please bring a valid photo ID, current insurance card(s), and copay to your appointment. Plan to arrive at least 5 minutes before your scheduled appointment to complete the registration process. If you are more than 15 minutes late, you may be asked to reschedule.
RDC requires adequate time to fill a cancelled new patient appointment. Therefore, if RDC does not receive notification to cancel a new patient appointment at least 48 hours before the scheduled appointment, there will be a $25.00 charge assessed. This charge must be paid in full before RDC will reschedule a new appointment.
